Instructions:
Form must be completed for each participant. Type the information on the form, print the completed form,
sign it and mail with check payable to "CROC" to:

Both PDF and Word versions are FORMS: You can type your entry information rather than clearly printing.
Check boxes can be clicked. The forms do not, however, calculate totals. Email forms are not accepted.

Western States Championships Eligibility: You are eligible to vie for the Western States Orienteering
Championship if you are a member of USOF, eligible for US championships, your primary residence is in
a Western state during at least 6 of the previous 12 months
(Alaska, Arizona, California, Colorado, Hawaii, Idaho, Montana, Nevada, New Mexico, Oregon, Utah, Washington,
Wyoming), and your primary club is located in a Western state.
WRE: All M21+ / F21+ entrants automatically qualify for WRE entry. Please provide your
WRE number on the registration form if you have one.
Relay Teams: Composition of relay teams and relay bib distribution will occur during PNWOF week before the
race day.
US Championships Eligibility: You are eligible to vie for US Individual Championship if you
are a USOF member in good standing and meet one of the following citizenship requirements: 1) Are U.S.
citizens and citizens of no other country; 2) Are in a permanent residence status commonly referred to
as "green card carriers"; 3) Hold dual (U.S.) citizenship and reside primarily in the United States; 4)
Hold dual (U.S.) citizenship, reside primarily outside the United States, and have not yet and agree not
to compete for a championship title in another country during the current calendar year; or 5) Are full-time
students in the U.S. holding a current student visa.
Junior: Competitors 18 years of age and under qualify for Junior fee structure.
Sport Ident Number: All competitive courses will use Sport Ident punching system. Please provide your SI number
on the form. If no SI number provided you will be assigned one and charged for the Sport Ident unit rental.
